A 15-year-old boy, MN, presented to the emergency room with progressive vision loss that started two months prior, worsening from night blindness to complete darkness. Despite initially being told nothing was wrong by an eye doctor, his condition deteriorated. MN had an extremely restrictive diet since toddlerhood, consuming only dinosaur-shaped chicken nuggets (up to 75 daily), candy gummies, low-cost chocolate bars, and a specific 3% juice drink for approximately seven years. Medical investigations ruled out common causes like premature birth complications, brain tumors, autoimmune conditions, infections, and heavy metal or methanol poisoning. A key clue was a prolonged blood clotting time, hinting at Vitamin K deficiency. The medical team shifted focus to nutritional deficiencies. Tests confirmed severe deficiencies in Vitamin A, Copper, and Zinc, all crucial for eye health. He was diagnosed with nutritional optic neuropathy and treated with IV vitamins and minerals. While his nutrient levels normalized, the damage to his optic nerves was irreversible, leading to permanent legal blindness.
